2015 Annual Report

Our Journey to High Reliability Continues…


The Joint Commission Center for Transforming Healthcare’s journey reached new heights in 2015 as we furthered our mission to transform health care into a high reliability industry. Collaborating with our participating organizations, the Center launched two new high reliability tools, published significant peer-reviewed publications, and began to offer high reliability in health care training programs.  All of our work starts with the patient. The goal of zero harm is what drives us. Why? Because, along with our participating hospitals and health systems, we believe high reliability in health care means consistent excellence in quality and safety for every patient, every time. The road to high reliability in health care is an ongoing journey. We hope that you will join us, because together, we can get to zero.

 

 

Our 2015 Accomplishments. . .

Oro™ 2.0 High Reliability and Organizational Assessment and Resource Library

In Q4 2015 the Joint Commission Center for Transforming Healthcare successfully launched its Oro™ 2.0 High Reliability Organizational Assessment and Resource Library—an online organizational assessment with resources designed to guide hospital leadership throughout the high reliability journey, specifically within the areas of:

  • Leadership Commitment
  • Safety Culture
  • Performance Improvement

The discoveries made throughout the Oro™ 2.0 process help organizations identify their high reliability maturity level and opportunities for improvement. A detailed summary report complete with resources help further the organization’s maturity level.

Take a Stand Against Patient Falls

In the third quarter of 2015, the Center launched its fourth Targeted Solutions Tool® (TST®). The NEW Preventing Falls TST is a unique online application that guides an organization through a robust falls project by:
  • Measuring the current state
  • Analyzing and discovering causes
  • Implementing targeted solutions
  • Sustaining and spreading improvements

Organizations who have used this systematic approach have reduced the rate of patient falls by 35 percent and the rate of patients injured in a fall by 62 percent. If the approach is translated to a typical 200-bed hospital, the number of patients injured in a fall could be reduced from 117 to 45 and avoid approximately $1 million in costs.

High Reliability in Health Care Training Programs

Leadership commitment to zero preventable harm, a culture of safety and effective process improvement are essential components on the journey to high reliability in health care. Achieving high reliability in health care through Robust Process Improvement® (RPI®) methodologies can lead to dramatic improvements in patient care, business operations, and financial performance. In 2015, the Joint Commission Center for Transforming Healthcare developed comprehensive High Reliability in Health Care Training Programs, which offer a disciplined, practical approach that brings new stakeholders to the table and sustainable solutions to the most persistent problems in health care that go far beyond the walls of the hospital.

Our Continuing Work

The Center and its participants—some of the nation’s leading hospitals and health systems—use a systematic, data driven approach to problem solving called Robust Process Improvement or RPI. The components of the RPI methodology—Lean, Six Sigma, and formal change management—have been used by hospitals to achieve breakthrough improvements in patient care and clinical outcomes. In keeping with its objective to transform health care into a high reliability industry, The Joint Commission shares these proven effective solutions with the more than 20,000 customers it accredits and certifies.
