The “Preventing Patient Falls: A Systematic Approach from the Joint Commission Center for Transforming Healthcare Project” describes the types of risks that lead to patient falls, the root causes for those risks and the solutions designed to help reduce them. Case studies feature individual hospitals and their experiences preventing falls.

While much work has been done in the hospital setting, patient falls continue to be a problem. As part of the Joint Commission Center for Transforming Healthcare’s ongoing work through its Falls Targeted Solutions Tool®(TST®): Several hospitals worked to identify specific factors that lead to falls and falls with injury, and develop targeted solutions to those specific contributing factors.  “Preventing Patient Falls: A Systematic Approach from the Joint Commission Center for Transforming Healthcare Project” is a new guide describing the types of risks that lead to patient falls, the root causes for those risks and the solutions designed to help reduce them. Case studies highlight individual hospitals and their experiences preventing falls.

can the TST tools for falls prevention be applied to the outpatient setting?
As more care is transferred to ambulatory care settings the focus on safety needs to follow to include programs and tools that address this population of patients.
