Preventing Avoidable Heart Failure Hospitalizations

Among older adults, heart failure is the most common reason for admission to the hospital. Through the Partnership, hospitals and community-based physician practices are working together to address the preventable factors that worsen the health status and quality of life for these patients. Preventing Falls With Injury

In November 2011, the Center for Transforming Healthcare launched its seventh project which aims to prevent falls that occur in health care facilities and result in injury to patients. Tens of thousands of patients fall in health care facilities every year and many of these falls result in moderate to severe injuries. These injuries can prolong hospital stays and require the patient to undergo additional treatment. Learn More

Safety Culture

In October 2011, the Center for Transforming Healthcare launched its sixth project which aims to optimize behaviors and practices resulting in an improved safety culture that reinforces and supports the prevention of patient harm. The Surgical Site Infection project is focused on patients undergoing colorectal surgery and is uncovering the major contributing factors to surgical site infections. Wrong site surgery is a serious and preventable adverse event that should never happen. Through the Partnership, the causes of wrong site surgery have been identified and solutions to put a stop to these preventable breakdowns in patient care have been developed and tested. Learn More
