About Reducing Sepsis Mortality Targeted Solution Tool

Following a comprehensive quality improvement project with a cohort of 5 hospitals across the country and a subsequent pilot that reduced sepsis mortality anywhere from nearly 25% for the cohort and from nearly 20% to over 50% for the pilot, the Joint Commission Center for Transforming Healthcare developed the Reducing Sepsis Mortality Targeted Solutions Tool (TST®). The methods and solutions embedded in the sepsis TST are based on the two projects coupled with the current measurement landscape and the experience of targeted solutions that worked for the project participants.

The Reducing Sepsis Mortality Targeted Solutions Tool (TST) is a web-based application designed to help health care organizations understand the barriers to sepsis recognition and sepsis protocol compliance so they can implement evidence-based solutions to reduce sepsis mortality.

The TST provides the foundation and framework for an improvement method that, if implemented as directed, will improve an organization’s sepsis mortality as well as help your organization better understand the issues around recognition and meeting the bundle element requirements. The leading United States hospitals and health care systems that developed the sepsis solutions have achieved and continue to show major and sustained reductions in their mortality rates.

The Sepsis module is targeted toward hospitals but can be used by any organization where sepsis impacts patient care such as Long-term care facilities. 

Not at this time. The data entry for sepsis is manual. However, we provide recommendations within the tool about who to best use to abstract your data, how much data you will need, and a timeline for data entry.

There is no hard requirement for the amount data entered. We recommend at least 30-50 patient records a month for 2 months of baseline and 30-50 patient records a month for 2 months of improve data. The more data entered into the tool, the more accurate the picture of what is happening within your organization.

Yes. The targeted solutions tool is flexible so you can run projects for specific areas in your hospital or you can run a hospital-wide sepsis project.

The targeted solutions tool is flexible so you can run a project however it best fits your organization. If your organization practices completing the 3-hour elements within the time of ‘sepsis’ then you can enter your sepsis criteria date/time and it considers that your time zero. However, if you no longer look at sepsis and are focused only on severe sepsis and septic shock patients, you can skip entering data for ‘sepsis’ and only enter date/times for severe sepsis and septic shock.

The duration of a sepsis project varies from organization to organization. Projects typically take about 6 months to complete.
