Through the Partnership, health care organizations across the country are identifying the causes of poor hand hygiene, which is a major cause of health care-associated infections, and are applying tested solutions to improve hand hygiene. Preventable Hospitalizations


Among older adults, heart failure is the most common reason for admission to the hospital. Through the Partnership, hospitals and community-based physician practices are working together to address the preventable factors that worsen the health status and quality of life for these patients.  Learn More



In October 2011, the Center for Transforming Healthcare launched its sixth project which aims to optimize behaviors and practices resulting in an improved safety culture that reinforces and supports the prevention of patient harm. A safety culture enables trust. It empowers staff to speak up about risks to patients, and report errors and near misses, all of which drive improvement.  Learn More




The Surgical Site Infection project is focused on patients undergoing colorectal surgery and is uncovering the major contributing factors to surgical site infections. Proven-effective solutions targeted to all of these factors are being developed for widespread use by health care organizations.  Learn More

The Center for Transforming Healthcare announces its eleventh patient safety project, which aims to prevent rates of venous thromboembolism (VTE). VTE is a major cause of morbidity and mortality in the United States. It has been estimated that up to 900,000 VTE’s occur per year resulting in approximately 100,000 deaths. Learn More
